Before we can begin, and in order to allow us to give you the most proffesional service we can, we require some preliminary information.

- I am aware that preforming a piercing is an invasive procedure that involves intervention in the body and may lead to complications.
- I am not under the influence of alcohol and\or drugs of any kind.
- I am currently in good health and do not have any medical condition that might endanger me or any of the studio staff members in any way by getting a new piercing.
- I am aware that in some cases it is recommended to down-size the jewelry after a certain period of healing time in order to ensure better and easier healing of the piercing and that down sizing jewelry does involve extra payment.
- אני מוותר על כל טענה ו\או תביעה על ידי או כל מי מטעמי כנגד עובדי ומנהלי Clean Cut או כל מי מטעמם ופוטר אותם מכל אחריות בכל הקשור לנזק או פגיעה הקשורים ישירות או בעקיפין לביצוע פירסינג או כל פרוצדורה אחרת בסטודיו ומאשר כי אני מבין שהטיפול בפירסינג מרגע עזיבתי את תחומי הסטודיו הוא באחריותי הבלעדית.
- אני מודע כי תכשיטי פירסינג הם בעלי סגירת לחץ \ הברגה\ ציר וכדומה וכי באחריותי הבלעדית לבדוק מפעם לפעם ולוודא שסגירת התכשיט עדיין מאובטחת שכן היא יכולה להיחלש או להתרופף עם הזמן גם בשימוש שבשגרה.
