I, The undersigned, here by approve that a piercing procedure be preformed on my underaged child, and confirm that I was informed by the studio crew about potential risks, what it may require to successfully preform and heal the piercing , and about proper aftercare instructions.

I hereby declare that I fully understand the potential risks involved and the information given to me, and that I take full responsibility for the piercing and the way it is treated and heals from the moment I leave the studio.
