By signing this document I approve that:

1. I am 16 years old or older.
2. I am interested in getting a new piercing done at Clean Cut and I approve the choise of jewelry and placement.
3. I am aware that preforming a piercing is an invasive procedure that involves intervention in the body and may lead to complications.
4. I am not under the influence of alcohol and\or drugs of any kind.
5. I am currently in good health and do not have any medical condition that might endanger me or any of the studio staff members in any way by getting a new piercing.
6. I am aware that in some cases it is recommended to down-size the jewelry after a certain period of healing time in order to ensure better and easier healing of the piercing and that down sizing jewelry does involve extra payment.
7. I hereby forgo any claim against any staff members of Clean Cut and release them From any liability regarding any damage or injury related to (but not limited to) piercing or any other procedure preformed at the studio and confirm that I understand that caring for my piercing once I leave the studio is my own personal responsibillity.
8. I am aware that piercing jewelery may be pressure secured, threaded, hinged etc. and that it is my sole responsibillty to check from time to time that my jewellry is secure as it may loosen or weaken from normal daily use.
9. Touching, removing or wearing any jewelry by yourself within the studio limits is strictly forbidden. (Yes, We know they are yours, and yes, even if they are fully healed, please follow our house rules).
